You should be able to get your immunization documentation from your physician’s office or other medical facility.

Indiana residents may also be able to get records through MyVaxIndiana. If you are asked for a PIN, contact your physician.

Non-Indiana U.S. residents, check with your state’s immunization registry program.

If you've not been vaccinated for all immunization requirements, students can visit the Health and Wellness Center.  Please call 317-497-6500 or visit www.mywebahead.com/uindy to schedule an appointment.

UIndy has partnered with Med+Proctor for immunization requirements. Follow the steps below to become compliant.


  1. Visit the Med+Proctor website.
  2. Click “Register for a New Account,” select University of Indianapolis, and use your UIndy email address to create either a standard Med+Proctor account or a Med+Proctor Pro account.
    1. With a standard Med+Proctor account (free), you do not retain access to your uploaded immunization documentation.
    2. With a Med+Proctor Pro account (additional charge), you receive lifetime access to your uploaded immunization documentation. A Med+Proctor Pro account is not required to comply with University of Indianapolis immunization requirements.
  3. Complete the required personal information and follow the directions provided. 
  4. Upload a copy of your medical documentation (in English) to your Med+Proctor account. Make sure your documentation is complete and legible.
  5. The documents will be evaluated by Med+Proctor trained health professionals who will either approve or reject each vaccine dose.
  6. Once the immunization forms are received and approved, you will receive an email confirmation.

Please note that you must submit this information even if you’ve already completed the UIndy Health and Wellness Center’s student health record and you may also be required to provide immunization information separately for your academic program. 


What are the next steps?

For those who are able to provide immunization records satisfying all requirements, your status will be updated to compliant in the Med+Proctor system. No further action is required.  

Students who provide records that do not meet the immunization requirements will be prompted to make any corrections. Until all appropriate records have been provided, a student is not compliant.

With the exception of online-only students, students seeking any other exemption must complete the Immunization Waiver Form. To apply for an exemption, the completed form—along with any additional required documentation—must be uploaded to Med+Proctor. 

Online-only Student Exemption

Immunization requirements only apply to students taking at least one in-person class. If all your classes are being taught online, you are exempt from UIndy immunization requirements unless required by your academic program. This is an automatic exemption based on your enrollment. There is nothing you need to do.

Medical Exemptions

Students are excused from providing medical documentation for one or more of the required vaccinations only upon provision of one of the following:

  1. You have immunity because you had the disease. A physician’s written statement is required to prove immunity.
  2. You have laboratory evidence of an immune titer.
  3. You are contraindicated to a vaccine. If a medical contraindication (e.g., allergy to eggs, pregnancy, reaction to vaccine, participation in a current sequence of immunizations, etc.) exists, a written statement from a physician is required to document each specific medical contraindication.

Please note that students filing a medical exemption may be required, for their safety, to leave campus if an outbreak of a vaccine-preventable disease occurs on or near campus. 


Religious Exemption

A religious objection does not exempt a student from immunization unless the exemption is made in writing and signed by the student. An Immunization Waiver Form can be returned in lieu of immunization records. Please note that students filing a religious exemption may be required, for their safety, to leave campus if an outbreak of a vaccine-preventable disease occurs on or near campus.


Philosophical/Personal Exemption 

A philosophical/personal objection is a non-medical exemption that permits individuals to decline vaccinations required by law because they personally object to them for reasons of conscience, morality, or general belief. Please note that students filing a philosophical/personal exemption may be required, for their safety, to leave campus if an outbreak of a vaccine-preventable disease occurs on or near campus.


How to Submit an Exemption

  1. Go to your Med+Proctor account or register if you have not already created one.
  2. Download the Immunization Waiver Form, fill it out, and upload it.
    1. For medical exemptions, complete the Immunization Waiver Form and submit a statement from your then or current medical doctor. If that is not available, then a lab titer document will suffice.
    2. For religious/philosophical/personal exemptions, complete and sign the Immunization Waiver Form.

When should I submit my immunization records? 

As soon as possible! You may think you have all the necessary documentation, but that may not be the case. The sooner you submit your immunizations the more likely you are to avoid interruptions in your ability to attend in-person classes in the event of an outbreak.

Why was my vaccine dose rejected?

If you go to your account on Med+Proctor, you should be able to see the issue notes left by the reviewer. The most common reasons a document would be disapproved are the following:

  • The document was too small or blurry to read.
  • The vaccine was administered outside the CDC's accepted timeline.
  • The document is missing the student's identifying information to prove it is the student's immunizations that were submitted.
  • The vaccine record is not in English and requires translation.
  • A vaccine is missing at least one of the required two doses.

If I claim a religious exemption or philosophical/personal exemption from one or all the required vaccinations, and an outbreak occurs on campus, what happens?

If there is an outbreak of a vaccine-preventable disease, UIndy students who have submitted waivers for any reason may be excluded from campus until the outbreak is over for their safety. 


I'm pregnant. I had some of my vaccinations as a child but not all, and in my current state, I should not take more immunizations. What do I do?

Ask your doctor or prenatal care provider to issue a statement confirming your pregnancy and the expected due date. You can upload that document to your Med+Proctor account to support your request for a medical exemption for the time being. The same precautions will apply to you during that time as with other kinds of medical or religious exemptions – should there be an outbreak on campus, and you have not been immunized, you will be advised to leave campus immediately, and not return until an "all clear" is given.
